If you are a circlemaker, this is an optional form you could fill in to provide with additional information in order to let all circlevault mail-list subscribers better identify your formation once you'll reveal your password.

If you want, please fill in and send it encrypted into the same rar file together with the graphical file (JPG, GIF) of your formation.
We highlight again that this form compilation is not mandatory and no field is required, but keep in mind that the more information you provide with, the more precisely everybody can identify your circle.
This is basically a form template, so feel free to add other infos you should consider important.
